Copyright Statement & Authorship Forms
Once your manuscript has been accepted for publication we require separate copyright statements to be signed by each author. If your manuscript includes more than one author, you are also required to send a single authorship statement signed by all authors.
Both of these documents must be in original form, and sent by mail to:
